11:  Replace broken fan with what?  
We will attempt to repair the existing ceiling fan.  In the event we can not fix the existing fan, we can at no charge replace the broken fan with a light fixture.  We can also replace the broken fan with a new fan however, you will be responsible for half the cost of the fan and installation.  Please press the back button and review the cost to replace a fan if you didn't read that portion of the ceiling fan information page.  

If we can not reuse or repair the existing fan, you are authorizing CP to replace your fan with:   
a light fixture at no cost to the Tenant.      a new fan at a cost to the Tenant specified on the ceiling fan information page.

If the condition of the fan warrants the need for a service request, we can not leave it in the property if it is not repairable.  If you prefer to leave with the fan that way it is, simply reset this form and do not submit it.
